The authors cite instances where Biden forgot the names of key aides, spoke incoherently, and experienced frequent memory lapses.

One of the most significant examples of Biden's decline is said to have occurred during his 2024 re-election campaign. According to the book, some members of Biden's cabinet questioned his ability to handle a national emergency in the middle of the night, noting that situations that might have been manageable three years prior would have proven catastrophic under Biden's leadership.

Biden's mental and physical decline is reported to have accelerated after the 2023 fall when he failed to recognize Jamie Harrison, the head of the Democratic National Committee. He also allegedly failed to recognize actor George Clooney, a long-time acquaintance, in early 2024.

Despite these concerns, the president's inner circle of advisors, including his wife, son, and long-time advisors, has maintained their support for Biden's continued presidency. This group, referred to as the "Politburo," prioritized loyalty over impartial evaluation of the president's health. According to the book, those not in the inner circle, such as campaign aides and cabinet members, were frequently kept in the dark about Biden's health conditions.

Biden's staff was said to be particularly concerned about his health during the 2020 campaign, with some recording hours of footage showing the president appearing confused and disoriented.

While Biden's age and health have become increasingly discussed topics, the debate over his decision to run for re-election continues. Vice President J.D. Vance remarked that, while wishing Biden the best for his health, it is crucial to consider whether Biden has the ability to perform the job due to his health concerns.
